If you book a lesson, we will need you to fill out a riding form. This provides details of your experience, health problems etc.  so that we can best help you. To save time at the stables, we have included the form on the website. You can print it out, complete it and bring it with you. Please do NOT post it to us.

The form is in two parts and can be printed out using the buttons below. Press the first button and choose to print it out. THEN press the "BACK" button at the top left hand side of the printing page. This will bring you back here to print page 2. Your printer is likely to warn that the margin settings are wrong. Ignore the warning, it just fits on a side of A4 paper.
